Who are we

The Paignton and Brixham Primary Care Network (PCN) Mental Health Team operates from Corner Place Surgery and consists of a dedicated group of professionals, including mental health nurses, an occupational therapist, and a non-clinical wellbeing coordinator.

If you are experiencing challenges with your mental health, your GP or practice nurse can refer you to our service. We offer mental health consultations to discuss your current situation and concerns.  We collaborate with both NHS and non-statutory services, enabling us to make referrals to adult mental health services and specialist services.

In certain cases, we can provide brief interventions to support individuals struggling with their mental health. However it is important to note that therapy is not part of our service. Nevertheless, we can guide you through  referral pathways to access appropriate therapy services if necessary.

Please be aware that we do not coordinate patient care, nor do we operate as a crisis or emergency service.

Who do we see

Our services are designed for individuals dealing with mild to moderate mental health difficulties. We welcome adults aged 18 and above, and we strive to maintain an inclusive environment by not imposing an upper age limit. However, we understand that each case is unique, and our service can be flexible to accommodate specific circumstances.

In an event of an acute crisis, such as experiencing suicidal ideation, thoughts of self-harm, or a severe relapse in a mental health conditions – such as Schizophrenia or Bipolar disorder, we have a separate referral pathway in place.

If you are currently under the care of the Community Mental Health Team at Chadwell, we kindly request that you directly contact the team on 01803 546470 (option 3) and ask to speak to the duty worker. Please note that the service operates Monday to Friday from 9am to 5pm.
